Qualifications

- Medicare/Medicaid credentials
- Unrestricted DEA license
- Board certified or board eligible as a Nurse Practitioner or Physician Assistant
- Two (2) or more years of professional experience in a primary care settingFellowships will count toward years of experience
- Fellowships will count toward years of experience

- Licensed as a Nurse Practitioner or Physician Assistant in the State of Colorado
